B.; Кривецкая, Анна Александровна; Савельева, Татьяна Александровна; Лощенов, Виктор БорисовичThe success of the surgical treatment of a tumor or obstruction of the esophagus with subsequent anastomosis application depends on the level of blood supply to the stitched tissues. Intraoperative assessment of blood flow is widely used in medicine and can be used as a diagnostic method that affects the outcome of surgery and reduces the frequency of postoperative complications for the patient. In this work, the assessment of blood supply during esophageal resection operations was carried out using two techniques sequentially: fluorescent diagnostics with indocyanine green and measurement of hemoglobin oxygen saturation by diffuse scattering spectroscopy in the visible wavelength range. The first method was used to assess the integrity of the vascular network structure in the area of anastomosis and blood flow through the sutured tissues, the second one – for local assessment of hemoglobin oxygen saturation in the investigated area. Conducted clinical study involved the participation of nine patients with malignant neoplasms (six cases) or esophageal obstruction (three cases). The presence of postoperative complications was compared with the measurement results. Anastomosis failure was observed in only one patient. According to the results of the study, with the use of the investigated method of assessing blood supply, there is a tendency towards a decrease in the frequency of anastomosis leaks (11.1 % compared with 21.4 %). Therefore, fluorescent diagnostics with indocyanine green and measurement of hemoglobin oxygen saturation using diffuse scattering spectroscopy were affirmed as methods that allow increasing the safety of surgical procedures by assessing the risk of postoperative complications, including anastomosis failures.

Infected wounds are not sufficiently susceptible to existing antibiotics, necessitating the search for alternative approaches to reduce the concentration of pathogenic microflora. Methods: This study describes an approach to the effective treatment of wounds by photodynamic inactivation or therapy (PDI/PDT) of antibiotic-resistant microflora under fluorescence control. For this purpose, laser and LED light (660ў??680 nm) and different groups of photosensitizers (PS) (1% solutions of methylene blue, aluminum phthalocyanine, chlorine e6 and nanocomposites containing these groups of PS) were used. The study included 90 patients with various wounds. Some patients were subjected to fluorescence diagnosis by laser spectral analysis before the PDT. Results: Positive results were achieved in 76 patients (84%, p andlt; 0.05). After the first PDT session, a decrease in the concentration of microflora was noticeable. By the third and seventh days, a significant to complete inactivation of bacteria was obtained. In all patients who were photo-diagnosed before PDT, a significant PS concentration decrease of more than 75% after PDT was obtained. Conclusion: PDT is an effective method for the inactivation of antibiotic-resistant pathogens, including in long non-healing wounds, contributing also to early tissue regeneration.

B.; Макаров, Владимир Игоревич; Скобельцин, Алексей Сергеевич; Лощенов, Виктор БорисовичBackground: Microvessels in tumor tissue play a crucial role in meeting the metabolic needs of transformed cells, controlling the entry of xenobiotics into tumor tissue, and regulating local inflammation that promotes metastasis. Methylene blue has photosensitizing properties and can also affect dysfunctional mitochondria. Methods: The study was performed on the primary culture of CECs. The cells underwent photodynamic treatment through 660 nm laser irradiation at a power density of 300 mW/cm2. The MTT, TMRE, and TUNEL assays were used to assess the survival, redox metabolism, mitochondrial activity, and apoptosis of CECs. Additionally, the metabolic activity of cells was evaluated using FLIM by measuring the fluorescence lifetime of NADH and FAD. Results: When CECs were incubated with MB, there was an increase in mitochondrial activity that was dependent on the concentration of MB. Additionally, mitochondrial activity increased when the CECs were exposed to 660 nm laser irradiation at an energy dose of up to 5 J/cm2. Following PDT, a slight shift towards oxidative phosphorylation was observed. Conclusions: In vitro application of MB accumulation or laser irradiation causes a shift in the redox status of CECs towards increased reducing activity, without causing any cell damage. However, the combined action of PS and laser radiation has the opposite effect on the redox status of cells, resulting in an increase in the oxidized form of FAD.

PDT with polycationic photosensitizers destroys CSCs and tumor neovascularization, and activates the desmoplastic reaction. These results open up new opportunities for increasing the effectiveness of treatment and reducing the incidence of relapses and metastases after PDT.

All rights reserved.Today, photodynamic therapy is one of the most promising minimally invasive methods of treatment of various diseases, including cancer. The main limitation of this method is the insufficient penetration into the tissue of laser radiation used to activate photosensitizer molecules,which makes it difficult to carry out therapy in the treatment of large or deep-seated tumors. In this regard, there is a great interest in thedevelopment of new strategies for photodynamic therapy using infrared radiation for excitation, the wavelengths of which fall into the “transparencywindow” of biological tissues. In this work, it was proposed to use upconversion NaGdF4:Yb:Er nanoparticles (UCNP), which absorbinfrared excitation and serve as a donor that transfers energy to the photosensitizer. Photosens and phthalosens were chosen as the mostpromising photosensitizers for the study. The aim of this work was to study the energy transfer processes between upconversion nanoparticlesdoped with rare-earth ions and photosensitizer molecules. in order to excite photosensitizers with IR radiation and carry out photodynamictherapy of deep-seated neoplasms. Using spectroscopic and time-resolved methods, it has been demonstrated that there is an efficientenergy transfer between upconversion particles and photosensitizers phthalosens and photosens. The calculated efficiency of energy transferby the Foerster mechanism was 41% for the UCNP + photosens system and 69% for the UCNP + phthalosens system. It has been experimentallyand theoretically proved that there is a binding of photosensitizer molecules with UCNP by means of surfactants, leading to a reductionin the distance between them, due to which effective nonradiative energy transfer is realized. The generation of singlet oxygen by the phthalosensphotosensitizer upon excitation by means of energy transfer from UCNP, excited at 980 nm wavelength of, has been demonstrated.
