    Fluorescence Diagnosis in Neurooncology: Retrospective Analysis of 653 Cases

    Objective: This study is to analyze fluorescence sensitivity in the diagnosis of brain and spinal cord tumors. Material and methods: The authors conducted a multicenter retrospective analysis of data on 653 cases in 641 patients: 553 of them had brain tumors and 88 spinal cord tumors. Brain tumor resection was performed in 523 patients, of whom 484 were adults and 39 children. The analyzed series was presented by 320 gliomas, 101 meningiomas, and 72 metastases. A stereotactic biopsy was performed in 20 patients and endoscopic surgery in 10 patients. In all cases, 20 mg/kg of 5-Aminolaevulinic acid was administered orally 2-h before surgery. All surgical interventions were performed with a microscope BLUE 400 to visualize fluorescence, while endoscopic surgery-with an endoscope equipped with a fluorescent module. Fluorescence spectroscopy was conducted in 20 cases of stereotactic biopsies and in 88 cases of spinal cord tumors. Results: Among adult brain tumors operated by microsurgical techniques, meningiomas showed the highest 5-ALA fluorescence sensitivity 94% (n = 95/101), brain metastases 84.7% (n = 61/72), low-grade gliomas 46.4% (n = 26/56), and high-grade gliomas 90.2% (n = 238/264). In children the highest 5-ALA visible fluorescence was observed in anaplastic astrocytomas 100% (n = 4/4) and in anaplastic ependymomas 100% (n = 4/4); in low-grade gliomas it made up 31.8% (n = 7/22). As for the spinal cord tumors in adults, the highest sensitivity was demonstrated by glioblastomas 100% (n = 4/4) and by meningiomas 100% (n = 4/4); Fluorescence was not found in gemangioblastomas (n = 0/6) and neurinomas (n = 0/4). Fluorescence intensity reached 60% (n = 6/10) in endoscopic surgery and 90% (n = 18/20) in stereotactic biopsy. Conclusion: 5-ALA fluorescence diagnosis proved to be most sensitive in surgery of HGG and meningioma (90.2 and 94.1%, respectively). Sensitivity in surgery of intracranial metastases and spinal cord tumors was slightly lower (84.7 and 63.6%, correspondingly). The lowest fluorescence sensitivity was marked in pediatric tumors and LGG (50 and 46.4%, correspondingly). Fluorescence diagnosis can also be used in transnasal endoscopic surgery of skull base tumors and in stereotactic biopsy.

Polycationic derivatives of synthetic bacteriochlorin with absorption in the near infrared range are promising for the creation of photosensitizers (PS) for antimicrobial photodynamic therapy. In the present work, the photophysical and antibacterial properties of PS based on tetracationic derivatives of synthetic bacteriochlorins: hydrophilic 3-Py 4 BC(EtBr) 4 Br4 in aqueous solution and amphiphilic 3-Py 4 BCHP 4 Br 4 in the dispersion of Kolliphor ELP were studied. Analysis of absorption and fluorescence in a wide range of concentrations has demonstrated low aggregation of the PS over the entire range. A high efficiency of photodynamic inactivation of Gram-positive Staphylococcus aureus and Gramnegative Pseudomonas aeruginosa, Klebsiella pneumoniae and Acinetobacter baumannii bacteria was observed.

    The engraftment dynamics evaluation of skin grafts via aluminium phthalocyanine nanoparticles using spectroscopic methods

    © Published under licence by IOP Publishing Ltd.The research and comparison of spectroscopic methods for assessment of skin grafts engraftment/rejection after the back cross skin transplantation of laboratory mice were carried out. The spectral analysis was performed via inflammatory reaction evaluation of the studied area. The inflammatory reaction intensity correlates with the fluorescence intensity of the aluminum phthalocyanine nanoparticles. The photodynamic therapy was carried out to improve the skin engraftment and reduce the inflammatory reaction.

    © 2019 Astro Ltd.In this paper we consider a method for researching the dynamics of blood flow in the cerebral cortex, on an optical phantom that reproduces the parameters of real human and mice brain structures, through the use of near and infrared ranges of laser radiation. For the investigation of real tissue we chose a laboratory mouse brain in vivo. An algorithm for non-invasive diagnostics of the degree of oxygenation was identified and optimal parameters of installation components were selected for taking information about hemodynamic indicators. Output was verified by the reference method for assessing oxygenation by degree of absorption of hemoglobin in the visible range, which indicates that data have a high correlation with classical methods. With further development, this algorithm can be used in various areas of research and diagnostics.

All rights reserved Due to the high recurrence rate after the glial brain tumor removal, methods of intraoperative navigation have a high relevance, providing the most complete removal of tumor tissues with maximum preservation of healthy ones. In this work a combined visualization method is proposed with an assessment of fluorescence and diffuse reflectance images. Fluorescence intensity of 5-ALA-induced protoporphyrin IX allows visualization of tumor cells, distribution of indocyanine green fluorescence helps to visualize the vascular system of the tumor, and parallel mapping of the degree of oxygenation demonstrate the hypoxic regions. The images were obtained in the near infrared range of the optical spectrum in order to maximize the optical probing depth in the window of biological transparency.

Currently, the most promising method for the study of pharmacokinetics of drugs with fluorescent properties is the spectral-fluorescent method. In this article, we propose an algorithm for expanding the dynamic range of the spectrum analyzer by automatically monitoring the maximum spectral density in the recorded fluorescence spectrum and automatically controlled changes in the accumulation time depending on this value, followed by compensation of the output signal with regard to this change, as well as hardware circuit solutions that allow this algorithm. Testing of LESA-01-"Biospeс" spectrum analyzer, upgraded using the proposed approach, was carried out on photosensitizer dispersions based on tetra-3-phenylthiophthalocyanine hydroxyaluminium of various concentrations (from 0.01 mg/l to 50 mg/l), approximately corresponding to the concentrations realized in the process of studying pharmacokinetics in calibration samples and tissues of experimental animals. The proposed solutions that implement the algorithm for recording fluorescence spectra with automatic change of accumulation time depending on the signal level, ensured a significant expansion of the dynamic range of the spectrum analyzer (up to 3.5 orders of magnitude) and improved accuracy in pharmacokinetic studies.

    Investigation of Ce6 accumulation and distribution in cell cultures of head and neck cancers

    © 2019 SPIE.Current paper presents the results of the chlorine e6 (Ce6) study on 2D and 3D models of FaDu cells culture. The 2D model or monolayer was used for investigation of Ce6 distribution within individual cells and their organelles. The 3D model or multicellular tumor spheroids were used for estimation of cells' metabolic processes by the investigation of the Ce6 fluorescence distribution within spheroid's layers and Ce6 fluorescence lifetime. It was shown that 3D cell cultures and Ce6 allows estimating the cells' metabolic processes better than in 2D monolayer cell cultures. Also, this model allows estimating the photodynamic effect depending on the proximity to the surface of different areas inside the heterogeneous 3D structure.

    Evaluation of vulvar leukoplakia photodynamic therapy efficiency by fluorescent diagnostics method with local «Alasens®» photosensitizer application

    © 2019 Elsevier B.V.Purpose: Using continuous-pulse irradiation mode application for performing photodynamic therapy sessions to introduce a new method, and conclude results of clinical research focused on vulvar leukoplakia photodynamic therapy efficiency in combination with topical aqueous «Alasens®» solution administration. Method: Seventy patients (average age of 61 years) diagnosed with vulvar leukoplakia disease (2018 ICD-10-CM Diagnosis Code N90.4) were examined. The following values represent doses of combined mode photodynamic therapy session: 12 J/cm2 pulse radiation dose; 3.5 J/cm2 continuous radiation dose. Non-invasive spectroscopic and visual control of drug accumulation in real time was carried out by fluorescence diagnostic method before and after each therapy session. Results: Single-therapy session efficiency was estimated by a fluorescent signal reduction in the pathological region after irradiation, and the direct correlation between photosensitizer photobleaching and disease regression was registered. Photodynamic therapy course included three procedures, with each session applied in 24 -h intervals, and when necessary, an additional course of therapy was applied 60 days afterward. Significant post-treatment results took effect after 2–3 courses with symptom presence reduced or fully regressed depending on the initial severity of the disease. Additionally, side effects and sequelae remained absent in all cases. Conclusion: The result of methods applied during the clinical research period indicate strong potential in utilizing such promising technology to contribute to the possible prevention of malignant transformation and the treatment of vulvar leukoplakia.

    © 2019 Astro Ltd.The treatment of infected, long-term, non-healing, complicated wounds of the skin and mucosa, trophic ulcers, pressure sores and ulcers of diabetic feet represents a serious problem, especially in the case of resistant and multi-resistant pathogens. Antibacterial photodynamic therapy can be a promising method of local infected foci treatment of such diseases. This work is devoted to the study of the possibility of photodynamic inactivation (PDI) of Pseudomonas aeruginosa bacteria in the form of biofilms with the help of new polycationic photosensitizers (PSs) based on octacationic phthalocyanine and tetra-and octabacteriochlorins (ZnPcChol8, (3-PyBrE)4BCBr4 and (3-PyEPy)4BCBr8). More specifically, this work aims to clarify the role of light irradiation in PDI with the participation of new PSs, which has not been fully studied, especially with respect to the comparison of different PS types.

    Trials of a Fluorescent Endoscopic Video System for Diagnosis and Treatment of the Head and Neck Cancer

    This article presents the results of intraoperative fluorescent diagnostics via the endoscopic system for assessing the quality of photodynamic therapy (PDT) of head and neck cancer. The diagnosis and PDT procedures were performed on the five patients with malignant neoplasms of the vocal cords, lateral surface of the tongue, and trachea and cancer of the left parotid salivary gland. Molecular form of chlorin E6 (Ce6) was intravenously administered with a 1.0-1.1 mg/kg concentration for PDT. Fluorescent diagnostics (FD) was conducted before PDT and after PDT procedures. Control of PDT efficiency was carried out by evaluating the photobleaching of the drug (photosensitizer). The method of intraoperative fluorescent imaging allows determining the exact location of the tumor and its boundaries. The assessment of photosensitizer photobleaching in real time regime allows making quick decisions during PDT procedure, which helps improving the quality of patients' treatment. The results showed the convenience of endoscopic fluorescent video system in various nosologies of head and neck cancer. Therefore, this diagnostic approach will improve the effectiveness of cancer treatment.
